Just happened to read this thread today and wondered if anybody suspected foul play?

making equipment that has interlocks or cannot be assembled wrong is not about removing personal responsibility. It is about good design that ensures that the manufacturer has done all they can to compensate for the inevitable moment when the holes in the cheese all line up. everybody can make mistakes if the circumstances are right. any professional engineer should plan for that moment and try to design out the possibility in order to prevent that mistake occurring in the first place or limit the severity of the outcome when that mistake occurs.

you would think this wouldnt even need saying....

(a general comment on some rb designs not specific or pointing fingers to whatever was used in this incident)

on a related note is it just me or has there been a significant reduction in death rate recently? 2015 looked like a good year. i recall a time when it was practically one a month

The weather was so sh*t globally in 2015 no one went diving.

(Easier to sit at home in your pants typing about rebreathers while stuffing your face with crisps and beer)
