What do you think caused these symptoms...

A friend dives a Meg. Uses Steramine. Cleans/sanitizes his loop and ISC neoprene counter lungs every dive. Runs a 1.0, normally. Bumps it up to 1.3/4 on deco.

Normal dive of 2-4 hours. Avg depth about 90 feet in Florida caves. Some O2 spikes and some dips, as expected. Usually sets the set point but sometimes runs manual.

550 hours on a breather. NO issues till 20 hours ago. No significant change in dive profiles. About 40 minutes of deco normally.

Issue: manifests 6-8 hours after exit. Extreme fatigue, chest hurts to fully inflate. Bit of a cough with a little phlegm.

Air diluent. Last dive was set at .8 to try and eliminate O2 as an issue. CNS clock was about a 75.

Any ideas? He has lowered the PO2 and still had the issue. He is now going to change the loop: lungs, hoses, ends, etc.

He is an anesthesiologist so he is familiar with the human body and not stupid.

    Any help would be appreciated.

    Bob

I'd pull the valves off the tanks too and inspect them for compressor contamination, just to eliminate one more thing.

How about trying a diluent with helium to see if it's related to WOB?

    Two excellent suggestions!

I advised the same: 30/20 or 21/35, his choice. He is an on-board diluent guy so it depends on how much helium he wants in his suit...or an inflation bottle.

Tanks were recently vipped but won't hurt to pop the tops, clean and get the new gas elsewhere.

He called since the post and is focused on a Steramine sensativity. Can't hurt to start anew.

Loaned him a set of lungs...a full, new loop actually.

How's he going to deal with the Steramine sensitivity?

Don't go changing all the things at once or you will never know the cause. But if he leaves everything else the same, pops in a new loop and comes up happy it immediately removes fills, valves, WOB and so on from the list of candidates.

A few happy dives on that clean loop start looking good, then flush it with steramine and see if symptoms re-appear.

One thing to think about, that this has nothing to do with diving. That the diving aggravates the condition.I would go see a good lung doctor that specializes in diving medicine.

Here's a thought: will a 2 hour couch dive show the same symptoms?

Might make it easier to hunt down if it does.

The couch dive is a good idea. I'll suggest that.

Another vote for seeing a doctor in case its the diving aggravating something else... as for blaming the cleaning- even the best cleaning isn't 100% so its possible its any number of irritants, hope he feels better soon.
