What if Scenarios

Marks' thread on High Risk CCR Practises got me thinking that it would also be useful to have a list of What if Scenarios that could be used to develop, and practise skills. Everyone knows that if you have experienced some kind of problem in the past that you handle it much more effectively and with less stress.

So lets start to post the "What if" ideas from your past training or actual experience of failures during a dive. I will then post a final list grouped in a manner that makes sense.
