I'm going to pay extra close attention to equalizing on the next couple of dives, put a little extra weight on so that i can keep a bit more air in the counter lungs (and the dry suit) and maybe try a different mask and see what happens. I'll report back in a week or so.

On the first dive I was very vigilant about the mask pressure and lung volume and had a complete absence of symptoms, our max depth was 110 fsw. Up to now, I think I have been taking minimum loop volume a little too seriously...i've been relying on the ADV to kick in to adjust the volume as i go down. This weekend I started using the manual dilluent valve to add more air instead, never relying on the adv. As I said on the first dive things were great. The next day, we decided to take along our cameras for the first time since starting rebreather diving... within a few pictures all the problems started up again, had a stint of blury vision, which went away after adding more volume to my loop, pissed off my ears after trying to equalize them with not enough volume in the loop (and from that point on they were "fussy")...all of this leading me to conclude that my issues are mask and loop volume/pressure related...oh, and yes compounded by increased task loading. I'm just surprised at how narrow the threshold seems to be, on OC I had a bit more lead time when i was getting a squeeze that could cause a problem. Since the tendency seems to be toward negative pressure, i'm pre-empting now and adding extra air to the counter lungs and less to my dry suit. I think i'm on the way to full resolution to this issue...thanks everyone for your input.
