Predive and post dive checklist

Does anyone have an electronic version of the meg pre/post dive checklist? I have a paper copy but would like to make a few changes as the version I had from my course assumes you do the o2 calibration once the unit is assembled rather than using the calibration kit. I'd also like to add a place to record o2 sensor readings.
