It was unfortunetly true that the foam under the scrubber didn't had a horse shoe shape, but was compltly round circle; and also unfortunetly the foam was first on top of the scrubber with then the grid on top, all of that with no doubt increasing the wob + the narcosis ...

Very sad, no course was done, maybe that would had save his life

Or at least talk to any experienced MK-15* diver who could walked him thru the correct set-up quickly...
