O2 sensors - What you DON'T KNOW - FAILURE!

I posted an incident with my O2 sensors failing some time back. I just now received a hard copy of the sensor failure report from Teledyne....and thought I'd share it with the comunity. It's fairly straight forward and has DEFINATELY made me more aware of checking out my sensors before diving or diving or installing them.

Attachments include:
1. my original O2 sensor failure report to Teledyne
2. teledyne's analysis and conclusion of the failure

Re: O2 sensors - What you DON'T KNOW - FAILURE!

Thanks for sharing. The teledyne recommendation is to use a machined polyethylene cell body rather than injection molded cell for the R22D to reduce cracking. Anyone know if a machined cell is currently available?

Re: O2 sensors - What you DON'T KNOW - FAILURE!

I've had cell failures from new cells. maybe we start a list of cell failures so we can Analise the figures our selfs, as this seems to be a major unknown with the systems we use

cell type r22. etc
manufacture date
failure date
symptoms of failure
mv if avaiable

any other info we need any one?
kind regards john routley

A question: do I read your report correctly, the sensors are part of the loop seal in the Meg? This would seem to mean that they carry a pressure difference across them (hydrostatic head due to CL bottom being below cells). I am surprised that the membrane could take that as a matter of routine, but it does seem to indicate a set of extra internal failure points. But, how can these be part of the seal when the p.c.b. would be exposed to sea water?

For the record, I have had 1 Teledyne R22-D d.o.a., no apparent cause. 1 APD 'Buddy' dead in 3 mo., 1 Buddy leaked - I blew 'condensation' off the membrane, and the immediate sting in my eye prompted to me to jump over the side and wash it out p.d.q. - be aware, this stuff is nasty. Another dead one. Teledyne K1-D - useless, all < 6 mo.

We have more like these on hand but lack of funds and time will be a limiting fcactor on when we can get to them. There are also reviews of failures in most evaluations done on different systems (Several in the Repository already).

Slightly off topic but interesting on recording of O2 sensor data.
