I'm starting to wonder exactly what came loose? Was it the knurled head connection between the head and the LP O2 hose or did the whole fitting come loose from the head?

Hmm. having read that again it doesn't come across very clear....any chance of a pic to identify exactly what separated from what?

Dave (Decodiver) mentioned the similar connection on the end of the hose to the APD ADV. While I don't always remove the connection to the ADV I do always check it is tight now after discovering, by chance, it was very loose (have added it as a line on my plasticised pre-dive checklist).

The other one to check is the spiggots for manual O2 and dil - discovered one of these loose and backing quietly out of the plastic body.
