Re: Intresting article in Underwater Technology

I found this article fascinating for a couple of reasons. First, using Fault Tree Analysis is a new and useful way to review CCR accidents. And second, it explores the man-machine interface and how it can contribute to accidents.

They have covered a lot of ground in a short paper, so there's lots of room to go forward from here.
