The theory at the time, but remember everyone was still shocked by the missile flying round the dive centre, was someone had put the wrong thread pillar valve in the tank i.e. NPT instead of M18 or something like that and it just came flying out under pressure. Seems the HSE have discounted that one? Interesting report.
Wondering when we will see a HSE advisory about people not O2 cleaning Apeks first stages properly.
