The only "failures" I've had have been due to my own stupidity/carelessness or natural wear & tear (sensors, etc) apart from two problems due to the previous owner of my KISS not maintaining it (i.e. knackering the valve seat on the BOV and not replacing the spring on the KISS valve).
Other than that my Rebreather has been more reliable than my OC kit. What I like about my KISS is that if anything does go tits on it then I can fix it generally with a trip to B&Q or Maplin.
