Defective & Dangerous Sensors

Have any other Prism divers experienced these defective and dangerous sensors? I received my Prism from SMI in September with 3 completely defective sensors. All 3 sensors read 0. The sensors I have since received "new" from SMI have read at or below 17mv. These sensors also fail to show 1.6 at 20 ft when doing an O2 flush. There are others in my locale that dive Prisms and have had similar issues with these sensors. When we have asked for new sensors, we are sent sensors that produce the same results.

Why are these sensors not being checked at AI prior to delivery to SMI? Why is SMI not checking these sensors upon arrival from AI?
Why is SMI not checking these sensors prior to selling them to customers.

These sensors are dangerous to use and the answers from SMI, has been to send out more bad sensors. Where does one go now?
