It's only happened once (so far).

When we were using the original Jetsam displays, which had the leads running through a length of tubing to keep the displays at ambient pressure, one of these tubes had developed a crack (I'd used it for years, and it was used when I got it). All positive and negative checks passed pre-dive, but once I was wearing the displays, the lead and tube were pulled far enough to open the crack.

This resulted in a slow flood, which I didn't notice until several minutes into the second dive of the day.

Until we changed to the Sub Sea Systems displays, I made it a point to wiggle the display leads while doing pre-dive checks after that.
