We should co-operate as an industry in getting a complete set of failure modes. This should not be a one-company affair. Safety is a joint responsibility in the industry and we have to be better at sharing information.
If you PM me with any extra modes you are aware of or post them here, we credit you as a contributor and of course update the document with that input. The goal is not impressing anyone; it is to avoid killing anyone. I would hope that eventually customers do look for public safety documentation from rebreather manufacturers. A simple way to use it, is to donate your input

and it will then also have your credits added to it.
If we have a common formal model and common database of top level rebreather failure modes, it reduces the work or companies to get 61508 approvals for rebreather related products.
