Later on in the document there are numerous references to a specific existing rebreather by way of failure type (battery bounce) components used (O2 and Dil manual injectors) etc that are clearly not part of the Open Revolution Submission, I do not understand where they fit in the analysis?
The next thing I spotted is that moany of the items are contradictary. For example- O2 Solenoid Stuck closed, the recovery suggested in flying PPO2 manually via manual injection (which of course is somewhat unit specific and doesn't afflict all rebreathers anyway)
On the very next page it is suggested that the manual injector is a failure point that should be eliminated!!
Perhaps I have missunderstood the purpose of this document, its seems to me a list of failures possible on the APD Inspiration, not an analysis of the Open Revolution submission (in which I imagine these issues are resolved)

The Vol 6 is a compilation of all known failure modes for rebreathers, against which the O.R. design is checked.
The objective in Vol 6 and 7 was to embed all the knowledge of what fails into a document and a model to create a complete filter against which the design can be checked.
Many are eliminated by design, such as battery bounce etc. The objective should be to eliminate all risks by design, but that is not possible in many cases.
Pls do PM me with Typos: the document has been checked several times, but it is a live document and some of the authors also do not have English as their first language.
